OUR HEALTH AND WELLNESS PROGRAM utilizes a dedicated Healthcare Coach to help patients with two or more chronic medical conditions maintain or improve their health.

HOW THE PROGRAM WORKS
Information Gathering On Initial Call
  • Assessment of your medical history
  • Discuss your current medications
  • Questions about your family and home life to better understand your needs
  • Discuss the possible need for financial assistance for medical expenses
  • Setting goals for your health
Follow-Up Calls
  • Healthcare coach checks in to discuss your health issues/needs
  • During the call a review of your medications will take place
Follow-Up Calls After A Hospital Stay
  • Healthcare coach reviews discharge instructions and reviews any new medications.
  • Schedules any necessary follow-up appointments

THE HEALTHCARE COACH
Within the Health and Wellness Program, the healthcare coach is the main driver of our success.
The coach works closely with patients and their entire healthcare team to:
  • Articulate goals
  • Establish personalized care plans
  • Coordinate all services necessary to achieve goals
  • Monitor treatment and prevention efforts
  • Facilitate patient referrals to specialty care when needed

THE MEMBERS OF THE HEALTH AND WELLNESS TEAM
The full care team may include:
  • Personally assigned physician
  • Physician Assistant
  • Specialty Physician
  • Nurse Practitioner
  • Medical support staff
  • Healthcare coach

HEALTH AND WELLNESS SERVICES INCLUDE:
  • Monthly contact by a healthcare coach to help manage your care
  • A personalized plan detailing your healthcare goals
  • Coordination of preventative care services
  • Medication management
  • Management of care transitions between healthcare providers
  • Enhanced opportunities for the patient and healthcare coach to communicate with practitioners
  • Access to providers and your plan 24 hours a day, 7 days a week

  • Facilitating discharges, transitions and follow-up procedures regarding hospitalizations, homecare services and nursing home stays
  • Monitoring patient satisfaction and instituting adjustments in program goals and needs based on consultations with and analysis by the healthcare team

INSURANCE COVERAGE
Many insurance plans including Medicare will pay for this program because it contains all the services of Chronic Care Management (CCM). The Multicare Health and Wellness program may involve a nominal fee, but in most cases insurance carriers will cover the cost of the copay.

GETTING STARTED
To enroll in the Multicare Health and Wellness program you simply need to sign THIS CONSENT FORM that will allow your provider and medical staff to provide the services to you. The consent form will also allow electronic communication of your medical information with other treating providers that have been involved in your care.
